No. Our Nurse Practitioners are Advanced Practice Registered Nurses (or better known as a Nurse Practitioners). They have Full Practice Authority (FPA) with the State of Illinois. This means based on their experience, years worked and continuing education, they do not need a collaborating physician to oversee their care.
Every hospital has their own hospital staff that will see your child if they get admitted. At this time, we are not on staff with any hospital, so you are free to go where you choose.
The hospitalist on staff at the hospital you deliver at will see your newborn daily until you are discharged. Once discharged, you can make your first newborn visit in our office.
If you have an urgent matter after hours that warrants immediate attention of the provider, you can call the office at (815) 322-9300 and it will connect you to the after-hours hotline. If it is an emergency, please take your child to the Emergency Room.
At this time, no we do not. Being a cash-based practice, and offering vaccines at a cash rate, would be costly for the patient. We can help you determine the schedule for your family and guide you to where you can receive them.”
Yes. You can call the office during business hours to schedule an appointment for your child. We will do our best to make sure you receive the best time available that fits your schedule and your needs.
Our contracted lab is HealthLab through CDH hospital, under Northwestern Medicine. We are able to bill lab services directly to your insurance. It is patient’s responsibility to look into coverage and ensure there is not a preferred hospital system they advise you to do diagnostic testing through. HealthLab processes through most major insurances and Medicare/Medicaid.

We are new patients here and had a wonderful first experience. Everyone in the office was so kind and friendly. Valerie was amazing with our daughter, very helpful and genuine in fielding all of our questions, and provided us with great resources. I am grateful for their whole child approach. So happy to have found this pediatric team!

We are new patients at Well-Rooted. We were greeted by the friendly receptionist who answered all our questions. Even though it’s a temporary location, it was very well organized and clean. We saw Allison for our visit. She was knowledgeable, friendly and took the time to listen and answer any of our questions. She put our children at ease and made the appointment very easy. Valerie and the entire team at well rooted have been an intergral part of my sons health journey. After visiting multiple Drs and not receiving a clear answer, I was at a complete loss on how to help my son. After our initial meeting with Valerie, I immediately knew we were at the right place! Not only did she take the time to listen but reassured both my son and I that we would get to the root cause of the issue. It hasn’t been a quick fix, however, after 6 months my son was feeling better and back to himself and I couldn’t be happier! Valerie didn’t just put a bandaid on the situation, but truly got to the root cause.
